Job Summary
The Outreach Patient Guide (OPG) primary job functions are to be responsible for initiating patient visits, scheduling patient appointments, entering/updating patient information on our computer system, providing courteous, efficient, and informal access to CVCH services and community resources. The OPG will support Outreach Coordinators by providing assistance during School Based Health Services Clinics, Community Clinics, Migrant Seasonal Farmworkers Clinics, and other Outreach related events. Provides interpretation and advocates and facilitates contact between patient/family and appropriate community resources.
Job Specific Competencies
1. Greets all patients during Outreach clinics upon arrival, orients patients to clinic services and provides interpretation when necessary.
2. Registers patients: collects demographics and updates data as needed; and assists them with forms if necessary.
3. Supports Outreach Coordinators with Outreach Clinic and Outreach Events.
4. Assesses patient needs and provides referrals to clinic services and community resources.
5. Schedules appointments and provides patients with necessary community information (maps, food banks, housing etc.).
6. Collects data, assists with data entry for tracking purposes and provides follow-up contact with patients either while at the Outreach clinical site or in clinic to record compliance, outcomes and address additional concerns
7. Obtains and verifies insurance information.

10. Follows Universal Precautions and Infection Control Standards and compliance with Joint Commission and HIPAA regulations.
Job Specifications
1. Education: High School graduate or GED required.
2. Certification/Licensure: Valid Washington State driver’s license required.
3. Experience: At least one year of office work. Experience working with the public, underserved population, agriculture and migrant/seasonal farm workers preferred.
4. Language Skills: Bilingual in English/Spanish Required
5. Essential Technical/Motor Skills: Must be able to listen and follow verbal directions in English. Knowledge of computer applications and equipment related to work. Must have basic computer and keyboarding skills and have the ability to enter data within company’s computer system; must demonstrate manual dexterity.
6. Interpersonal Skills: Ability and willingness to work with diverse individuals. Requires verbal, written and listening communication skills, especially in the language of the community; ability to teach; ability to work on a team, work with diverse groups, ability to develop rapport and trust; ability to work in a hectic environment.
7. Essential Physical Requirements: Must be able to lift and carry up to 30 pounds occasionally, using proper lifting techniques and with assistance if needed. Must be able to stand/walk between 4-8 hours per work day; Must be able to push/pull with arms with force, lifting, reaching, kneeling, bending, stooping, pushing, and pulling occasionally; Must be able to use hands and fingers to grasp and manipulate equipment and materials needed to perform essential job duties on a frequent basis. Ability to work indoors and outdoors and travel to various locations for performance of duties.
8. Essential Mental Abilities: Must be able to follow verbal directions in English/Spanish. Ability to communicate effectively with CVCH employees with a friendly attitude and support. Ability to work in an atmosphere that may become chaotic and stressful. An intimate understanding of the MSFW culture, familiarity with the local agricultural region, understanding of migrant health centers throughout the western migrant stream, issues of migrant health, migrant work and living conditions. It also required an ability to quickly learn or already possess a thorough understanding of clinic services and service gaps.
9. Essential Sensory Requirements: Essential sensory requirements include the ability to: read computer keyboard, monitor, and documents; hear, recognize, and assess verbal presentations of individuals; receive and convey detailed information orally, by telephone and in person; convey accurate and detailed instructions by speaking to others in person and by telephone in English and/or Spanish.
10. Exposures: Outdoor field work includes possible exposure to various domestic or wild animals. Exposure to heat, cold, wind, dust and precipitation depending on prevailing weather conditions.
