**MDS LPN Coordinator Summary: **

As the MDS LPN Coordinator, you'll serve as the Resident Assessment Instrument (RAI)/Minimum Data Set (MDS) Coordinator for our center. Your responsibilities will include reviewing and processing MDS assessments in accordance with CMS and LTCI regulations and guidelines.

**MDS LPN Coordinator Responsibilities: **

Your impactful contributions will include:

- Coordinating the development and revision of resident Interdisciplinary Care Plans based on RAI/MDS information

- Collaborating with nursing caregivers and other disciplines to ensure accurate care plan goals

- Effective communication with various disciplines to facilitate timely completion and transmission of RAI/MDS

- Preparing accurate reports on Quality Measure/Quality Indicators (QM/QI) from RAI/MDS data

- Active involvement in Quality Improvement initiatives

- Implementing educational plans to address changing program or service needs
