Empath Home Health, a member of Empath Health, is seeking a PRN Registered Psych Nurse. The Registered Psych Nurse helps provide each Avidity Home Health patient with nursing services to include behavioral health care services and will be assessed for emotional, behavioral, and psychosocial functioning.

The PRN Registered Psych Nurse:

• Performs initial and ongoing comprehensive assessments of the patient’s needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points.
• Initiates the plan of care and revise as necessary.
• Provides nursing services and/or treatments indicated on plan of care and evaluate the effectiveness and outcomes of care.
• Counsels and educates the patient and family on disease process, self-care techniques and prevention strategies.
• Initiates appropriate preventive and rehabilitative nursing procedures.

Requirements:

• Current licensure as a Registered Nurse in the respective state.
• One (1) year home health care experience preferred.
• Specialized training and/or experience beyond the standard curriculum required for a registered nurse. These Registered Nurses meet the training and experiential requirements of the State of Florida and/or the Regional Home Health Intermediary.
