We're seeking a dedicated Social Worker / Case Manager to continue the growth of our Social Work Case Management team at Emory University Hospital. As part of our team you will be responsible for coordinating clinically complex patient care needs across the health care continuum.
You will assess patients for transition needs including identifying patients at risk for readmission. The Social Worker / Case Manager will appropriate the facilitation of high quality clinical and cost outcomes; negotiate, procure and coordinate services and resources needed by the patients in a timely and effective manner.
You will collaborate with Utilization Review in order to identify and address barriers to timely and effecient care delivery and reimbursement.
