Duration: 13 Weeks
SHIFT/HOURS: Days 7:30am-4pm
Guaranteed Hours: 40hrs
Responsibilities:
- Organizing, facilitating, and evaluating care options to best meet an individual’s mental, physical, and emotional health.
- Achieving holistic client well-being by advocating for their needs, identifying providers, and ensuring services are cost-effective.
- Evaluating clients, identifying their needs, and developing plans to help them.
- Connecting clients with needed services and resources and evaluating their progress over time.
- Preparing reports, writing grants, and advocating for clients.
