Job Description:

Embracing HospiceCare is seeking a highly motivated and energetic Registered Nurse Case Manager with at least 1year of hospice or home health experience to cover Middlesex County.

The Hospice RN Case Manager establishes a professional and caring relationship with our patients and families, support staff, physicians,discharge planners, social workers, facilities and other health careprofessionals by providing hands-on care, on-going clinical management and educationregarding hospice care.

Job Requirements:

• Registered Nurse with a current NJ RN license
• Current vehicle liability insurance and license
• Excellent people, verbal and written communication skills
• Ability to adjust to daily needs of patients
