Where do you fit?

To be a successful Crossover Support Worker you must become able to do the following always:

1. Ensure a trauma informed focus on creating a safe and supportive environment for staff and those served through knowledge, sensitivity, respect, and a focus on resilience and recovery.
2. Provide services that result in timely and safe achievement of company and state outcomes relevant to the position.
3. Work with youth who move between the juvenile justice and child welfare systems utilizing the Crossover Youth Practice Model (CYPM) to address the unique needs of the youth.
4. Assist the Crossover Permanency Specialist with making referrals, monthly contacts, and ensuring documentation is complete.
5. Assist families with identifying, locating, and accessing basic resources and community support systems.
6. Ensure documentation is completed and/or upload timely according to company policy and procedures.
7. Transport children and families. Travel in state to assure client-family contact as needed.
8. Follow Company Policy and Procedure and operate according to the Company’s Program Practice Guidelines.
