Elizabeth Seton Children’s is Hiring a Social Worker - Pediatrics - Full Time Day M-F Near Yonkers, NY

Elizabeth Seton Children’s Center is a pioneering provider of care, education and hope for children with some of the most challenging medical complexities any person can face. Featuring the largest long term residential medical center of its kind in the United States.

This is a full time Monday-Friday 40 hour work week with rotating late night Thursday until 6p.

Our Social Workers provide medically related social work services, which support the physical, mental and psychosocial well-being of each resident and family.

Responsibilities will include:

  • Assists families with the pre-admission process, facility tours, and Admissions paperwork.
  • SW orients the family to the Pediatric Center and facilitates the admission process with the interdisciplinary team. SW reviews the Admission Packet and the Resident Bill of Rights.
  • SW assists resident/family with adjusting to the Pediatric Center and acts as a liaison between the Center and resident/ family throughout their stay.
  • SW interviews the family and any collateral contacts to complete the initial psychosocial assessment and updates as needed.
  • SW assesses and assists the resident and family with governmental benefits, such as SSI and Medicaid as well as school placement, guardianship and other legal/social needs.
  • Participates as an integral member of the inter-disciplinary care team and makes psychosocial recommendations to assist in the resident/family’s overall care and well-being.
  • Documents in both the EMR and the required Care Plan sections of the MDS according to regulatory guidelines and timelines.
  • SW plays a key role in assessing for behavioral and mental health concerns and refers to the appropriate internal or external providers or services.
  • Advocates for the physical, emotional, social, educational and spiritual needs of the resident/family.
  • Provides casework, family sessions, crisis intervention and group work services to families and residents as needed. SW encourages family participation in care plan meetings and keeps family/legal guardian informed of child’s progress.
  • SW works closely with the child welfare system and assists the IDT when concerns of abuse/neglect arise. SW follows all protocols as a state mandated reporter.
  • In collaboration with IDT, SW oversees discharge planning process. SW confers with insurance case managers as needed to coordinate continued stays and discharge planning.
  • Keeps informed of community resources and makes appropriate referrals to support the psychosocial needs of both the resident and family.
  • Facilitates and encourages resident/family involvement in group and educational programs at the Pediatric Center.
  • Provides palliative care and end of life services to residents and families. Follow up with families for bereavement support.
  • Attends and participates in the annual mandated training program, and other workshops and training, either in-house or off-site.

  • Other related duties as may become necessary or as directed by your supervisor, department director, and/or administrator.

Qualifications

  • MSW from an accredited University in Social Work preferred

  • LMSW or LCSW

Experience

  • Two years of post-graduate experience, including work with families and children, preferably in a pediatric setting.
  • Experience or exposure to medically complex children and their families.

Salary Range:

$62,400/Yr to $68,000/Yr
