Required : DANB Certified

- Have you had a PPD Skin Test (TB) within the last 12 months with a negative result? Must be able to provide a copy of the results.
If you do not have a copy, please answer no. (yes)
Are you vaccinated against HepB or have you had a titers test done recently with a negative result? Must be able to provide a copy of vaccination records or titers test results.
If you do not have a copy, please answer no. (yes)
- Are you DANB certified? (yes)
- Have you already spoken with your Relode recruiter on the phone to go over job details? (yes)
- Can you work all the days / hours listed in the job description? (yes)
