Position: Program Manager GLE/SIE
Description of the position: Manage and oversee the day-to-day operation and overall service delivery of Assigned Group Living Environment (GLE) or Supported Independent Living Environment (SIE). Coordinate and oversee all case management related services for persons/clients served in the program. This includes assisting persons served with managing income, insurance and other entitlements; assisting in accessing other services and community resources and scheduling/coordination of all medical and
psychiatric appointments; participating in the Assessment process and the development of Treatment Plans; maintaining progress notes and all documentation related to the Treatment Plan and contact with the client; providing interventions to persons as enumerated in their current Treatment Plan to assist clients with working towards achieving goals and developing skills in order to move into a more independent setting. Duties also include assisting clients with identifying, building, and utilizing coping
skills as needed; participate in risk assessment, crisis planning and prevention, and in the development of a safety plan with the client as needed; implementing crisis intervention as necessary which encompasses but is not limited to: use of de-escalating modes of communication/interaction to ensure client;s safety, stability, mental status, and physical health; utilize supervisor and team members for consultation; communicating all pertinent information with attention to clinical observations.
Requirements: A bachelor's degree in psychology, social work, public health, or a related field and three years of experience or a master's degree in psychology, social work, public health, or a related field
