JOB TITLE: Memory Care Program Coordinator

REPORTS TO: ALSA

SALARY CLASSIFICATION: Hourly

General Description:

Proactively assists residents and family members in identifying appropriate internal and external resources to maintain their highest level of independence, physical, mental, emotional, and spiritual health for those with memory care issues and/or concerns. Coordinates support services with the residents, their family members and the Director of Assisted Living Services.

Develops, implements, and assesses programs to enrich the lives of people with cognitive/memory impairment. Oversees the overall operation of the day program provided. Assists and implements additional memory care services to residents in EPP.

Essential Job Functions:

  • Oversees the day-to-day operation of the memory care day program (The Palms)
  • Provides information and markets the program to EPP residents.
  • Inventories and orders needed supplies for the day program. Follows budgetary guidelines for the program.
  • Tracks attendance of participants and provides needed information to (finance department/ALSA) for billing purposes.
  • Provide information regarding participation, interaction, and engagement of participants to designated family members.
  • Works closely with the Life Enrichment Director and other LE associates to coordinate use of spaces and programs.
  • Develops a weekly calendar of events for the day program.
  • Coordinates with transportation for outings 2 times monthly
  • ALSA, provides ongoing assessments of participants to determine appropriateness of the program for individuals.
  • Attends weekly ALSA meeting to discuss ongoing needs of EPP residents.
  • Assists with administering SLUMS testing as determined by the ALSA.
  • Support memory care screenings by marketing, administering, and following up as needed.
  • Maintain resource library by evaluating current material and researching additional resources.
  • Conducts presentations, training and courses related to brain health and memory for residents of EPP.
  • Conducts in-services for associates on topics related to memory care.
  • Researches and uses best practices in the MC field.
  • Provides input regarding future memory care services to be offered at Elim Park.
  • Monitors and tracks participation to determine additional staffing needs. Collaborates with the Administrator to adjust staffing as needed.
  • Assesses the needs and provides input for additional supportive service programming in collaboration with residents, management, and local community service providers.
  • Establishes collaborative relations with provider agencies, support services, community resources.
  • Works with consultants and MC experts.
  • In consultation with the Administrator/ALSA Director, hires, trains, disciplines, and terminates department associates in accordance with company policy.
  • Serves as liaison for residents with the ALSA and participates in QA committee.
  • Collaborates with ALSA Director in the orientation and training of MC/ALSA staff.
  • Meets with all new residents of EPP to provide information about memory care services and establish a relationship.
  • Responds to emergency situations and alerts medical help when necessary.
  • Addresses and follows up on all resident's concerns or incidents in timely manner.
  • Contacts family members during times of emergency and other concerns.
  • Develops and maintains care plans and other background information to provide appropriate services to residents.
  • Recognizes resident change of condition and initiates proper intervention.
  • Performs other job-related duties or responsibilities that may become necessary or as directed by the Administrator/ALSA Director.

Elim Park Place Apartments, Health Care Center, indoors and outside.

Entry Level Qualifications:

  • Bachelor’s degree in social work, nursing or related human service field and minimum of 3 years’ experience in a social service delivery system dealing with issues and coordinating services to persons with memory care issues.
  • Prior supervisory or management experience.
  • Applies existing guidelines and procedures to make varied decisions within a department. Uses sound judgement and experience to solve moderately complex problems. Must have the ability to remain calm in stressful situations, to be flexible, to work with many interruptions and have skill in multitasking.
  • Compassion and empathy for the geriatric population.
  • Must be a team leader and a team player. Able to work with others.
  • Knows infection control policies and procedures, standard and universal precautions.
  • Occasional weekend, evening or night coverage if needed.

Physical Requirements (with or without reasonable accommodation):

  • Prolonged, extensive, or considerable standing/walking/sitting.
  • Lifts, positions, pushes, transfers residents.
  • Manual dexterity and mobility
  • Intermittent exertion when performing care.
