Elevation Infusion Services is seeking Per Diem Home Infusion Registered Nurses to join our Team! You will be responsible for the assessment, diagnosis, treatment evaluation and education of patients receiving Infusion Therapy and/or other nursing services in accordance with the care matrices and best practices in the home or alternate site setting. Position Summary:

As Home Infusion Nurse your role is to provide a positive, safe, and professional patient care experience. You are responsible for the quality and accuracy of services provided and will perform all tasks with the utmost care for the patient’s wellbeing. You will thoroughly review patient orders, medications, and health records and will administer parenteral medications in accordance with the treatment plan. You will need to assess and monitor a patient’s health status and must be able to detect changes in symptoms or health status and know when additional intervention is necessary. All patient notes must be thoroughly and accurately documented to maintain ongoing compliance with the Company. You must maintain a professional appearance and conduct.

Major Responsibilities:

● As a trusted healthcare provider, we have an undeniable responsibility to protect the health and safety of our patients, customers and team members: COVID-19 vaccinations are required as a condition of employment for patient and customer facing team members as well as all supervisor level and above leadership roles.
● An Infusion Nurse is a licensed clinician who coordinates and provides direct patient care in the home or alternate treatment site to ensure patient safety with continuity and compliance under a physician’s plan of care.
● Works under the direct supervision of the designated supervisor.
● Initiates, develops and implements nursing plan of treatments; evaluating patient progress towards goals.
● Organizes and participates in the provision of direct patient care, performs treatments, administers medications, and educates patients and families/caregivers.
● Modifies plan of treatment in response to changing patient status or physician orders to achieve established or revised patient care goals.
● Assesses patient needs and physical status at each skilled visit through health data access and patient interview.
● Re-evaluates patient needs through physical reassessment, response to therapy, and supplemental physician orders.
● Obtains and clarifies physician orders for plan of treatment revisions, informs physician promptly of significant changes in patient's condition, and provides written summary to physician within supplemental order.
● Provide skilled care to patients of varying ages and medical conditions, as outlined in the plan of care and in accordance with physician orders and the plan of treatment.
● Establish, secure and maintain vascular access devices as prescribed, including (but not limited to) peripheral IV (PIV) placement, PICC and tunneled central venous catheter, dressing changes, and implanted port access.

● Effectively and timely communicates with Elevation Infusion team, medical providers, patients and families/caregivers to facilitate continuity of care.
● Prepares clinical documentation in real-time during visits and submits to the appropriate Elevation channels to comply with established time lines for billing optimization.
● Completes all documentation legibly, and applies approved abbreviations and documentation error correction practices per Elevation’s policy
● Coordinates discharge planning and prepares discharge summaries with patient instructions and thoroughly reports patient care needs, progress and goals when transferring care.
● Demonstrates compliance with agency operations, Elevation’s policies and procedures, professional standards, local, state, federal regulations/guidelines, and accreditation standards.
● Maximizes work efficiency through the use of computers and other automation technologies to validate plan of treatment orders, communicates patient care provided, and follows assigned visit schedule.
● Completes Daily Activity Record paper or electronic notes (via care management system) accurately and legibly according to agency submission requirements at the end of each work day.
● Complete all required clinical documentation thoroughly and timely, including nursing plans of care (POC), plans of treatment (POT), visit notes, clinical summaries, medication profiles, discharge summaries and any updates provided to members of the patient's care team.
● Responds promptly and appropriately to patient requests.
● Initiates emergency procedures as necessary.
● Accepts accountability for own practice through ethical and professional conduct.
● Follows established programs and practice within policies and procedures reflective of Elevation’s mission, values and objectives.
● Observes legal and ethical guidelines for safeguarding the confidentiality of patient and proprietary Elevation Infusion Services information including adherence to HIPAA regulations.
● Speaks knowledgeably about Elevation’s scope of services.
● Attends and completes required training modules, in-services, and continuing education to maintain competency and professional licensure for demonstrated knowledge regarding the care and management of patients in the home and/or alternate care settings.
● Participates in multidisciplinary team conferences and provides mentoring to other nurses for orientation and supervisory activities as assigned.
● Participates in Elevation Infusion Services on-call responsibilities.
● Actively participates in process improvement program and assists Supervisor or Nurse Manager with chart audit functions as applicable.
● Performs other related duties as directed by supervisor

ALL DUTIES AND REQUIREMENTS MUST BE PERFORMED CONSISTENT WITH ELEVATION’S PERFORMANCE STANDARDS.

Qualifications:
● Six (6) month probationary period.
● Proof of eligibility to work in the United States.
● Graduate of an accredited school of nursing and at least two (2) year of relevant
experience as a registered nurse or recent graduate with prior experience as an IV
Certified LVN/LPN with one-year professional nursing experience.
● Required license in Registered Nursing as granted by the state(s) of practice
● Current CPR certification.
● Demonstrated competency in patient care standards required for safe delivery of
services and infusion skill sets applicable to agency programs and service needs
● Advanced certification and training as applicable.
● Required license to operate a motor vehicle in the state of practice with access to a
vehicle for business travel with proof of liability insurance.
● Basic knowledge of computer operating systems and software applications with the
ability to apply knowledge in the effective use of nursing technology tools to
communicate and document care provided.
● Willing to travel up to 100% of the time for business purposes
● Valid Registered Nurse license in states/service territories Elevation Infusion Services operates OR a reliable means of transportation.
● Progressive nursing management experience with demonstrated success.
● Current regulatory, accreditation, and relevant experience with infusion home health services.
● Experience in clinical teaching within an education program or as clinical preceptor/unit teacher or equivalent teaching experience.
● Applicable clinical certification.
● Ability to work, at a minimum, one weekend per month.
● Experience administering Infusion therapy in a Home Setting Knowledge of and experience with Microsoft Office and electronic medical records (EMR) programs.
● Knowledge of the Infusion Nursing Society (INS) standards of practice, policies and procedures.
● Knowledgeable of HIPAA patient privacy and data security requirements, OSHA employee protection regulations, and accreditation standards for home health and home infusion pharmacy (or other relevant accrediting organizations)
● Excellent organizational and critical thinking skills.
● Effective interpersonal communications, both written and verbal.
● Candidates must be able to perform all essential job functions with or without reasonable accommodation.
● Willing and able to work with an interdisciplinary team of professionals including pharmacists and technicians, physicians, pharmacies, manage service providers, and other licensed prescribers, registered dietitians, home health agencies (HHAs), sales representatives, and others as needed to provide comprehensive care and support of patients and caregivers.
● Willing and able to work independently in home or alternate-site settings.
● Willing and able to assume a flexible work schedule in meeting attendance, overtime, after-hours on-call, and other reliability requirements of the position, if applicable.

Infusion Core Competencies Requirements:
● Critical thinking skills
● Phlebotomy
● IV placement, including peripheral (PICC- peripherally inserted central-line catheter), Central
line (CVC), and Midline (Midline Catheter Placement).
● Dressing changes
● At least two (2) years of relevant Infusion experience

Skills:
● Demonstrated interpersonal and customer service skills necessary to interact with all internal and external parties.
● Demonstrated competency in patient care standards required for safe delivery of services and infusion skill sets applicable to agency programs and service needs. Advanced certification and training as applicable.

Physical Requirements:
Required to stand, walk, sit, talk and hear; required to use hands to operate vehicles and office equipment; must be able to occasionally lift and/or move up to 25-30 lbs.; specific vision requirements for this job include: close, distance and peripheral vision, and the ability to adjust focus, reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of this position without compromising patient care.
