Hospice Nurse/Case Manager

Job Summary/Purpose:

The Nurse/Case Manager is responsible for identifying patient/family needs and for providing skilled nursing assessment, planning and supportive care and appropriate services in accordance with the attending physician’s orders and the interdisciplinary group plan of care.

Organizational Relationship:

Reports directly to the Clinical Supervisor or designee.

Qualification & Experience:

· Possess a current license to practice as a registered professional nurse in the IN.

· Minimum two years of varied work experience as a professional nurse with preferably one of the two years in an acute, oncology, Home Health and/or Hospice setting.

· Demonstrated ability to assess and respond to the needs of patients and families in varied settings Ability to turn, reposition, and assist patients to transfer and ambulate.

· Must be able to bend, stoop, push and pull Must be able to lift a minimum of 25 pounds.

· Position requires manual dexterity sufficient to provide hands-on care to the patient under variable conditions and settings.

· Must have keyboarding and computer skills.

· Position requires normal ranges of hearing and vision.

· Must be free of communicable diseases (TB).

· Must be able to communicate clearly in English verbally and in writing.

· Demonstrated skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles.

· Possess a valid IN driver’s license, valid auto insurance, and an automobile for field use.

Responsibilities & Duties:

· Assume primary responsibility for a patient/family caseload that includes the assessing, planning, implementing, and evaluating phases of the nursing process.

· Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family.

· Maintain regular communication with Nurse Supervisor to review caseload.

· Maintain regular communication with the attending physician concerning patient/family care needs and response to interventions.

· Obtain data on physical, psychological, social, and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care.

· Seek input from other group members regarding the patient plan of care to obtain additional knowledge and support.

· Maintain up-to-date patient records using computerized medical record system so that problems, plans, actions and goals are accurately and clearly stated, and changes are reflected as they occur; documentation is completed within established agency guidelines and to meet regulatory requirements.

· Instruct primary caregivers, volunteers, and employed caregivers to provide care as indicated.

· Meet regularly with hospice nursing staff to review problems or unique issues from caseloads, share professional support, and exchange feedback aimed toward enhancing professional growth.

· Attend Interdisciplinary Team meetings and facilitate discussion of issues from caseload for full staff discussion, consultation, and evaluation.

· Supervise and document observance of the hospice aide delivery of care every month. Inform the Nurse Supervisor of unusual or potentially problematic patient/family issues.

· Assist other nurse/case managers including assuming primary responsibility for patients when necessary.

· Share in on-call rotation, providing 24-hour, seven-day-a-week coverage to patients/families as necessary.

· Provide appropriate support at time of death and perform bereavement assessment if requested. Participate in the agency’s Performance Improvement program as appropriate.

· In states where allowed, Hospice RN shall pronounce the patient according to Agency policy, complete the necessary documentation and notify the MD and family.

· Participate in Hospice’s orientation and in-service training programs for professional staff.

· Participate in agency and community programs as requested to promote professional growth and understanding of hospice care.

· Demonstrate familiarity with policies of the agency and rules and regulations of State and Federal bodies which aid in determining policies.

· Perform specific assignments as directed.

Job Type: Full-time

Pay: $80,000.00 - $88,000.00 per year
