Position :

• Start Date : End of June / early July
• Schedule : Monday Thursday 8 am 5 pm (1 hour lunch)
• of Dentist : 2 (Dentist in practice relocating)
• Support Staff : 3 DAs, 1 Hygienist, Dental Receptionist
• of Ops : 7 Charis 1 Hyg 6 Production
• Procedures : Teeth cleaning, Fluoride & sealant treatments, Treatment plans, Fillings, Dental X-rays, Extractions, Special Needs Care
• Patient Ages : Primarily Adults, Ability to see more / less Peds if desired
• Census Daily / Annual : 8-10 Hyg, 10-14 production

Offering :

• Base Salary : $185-195k (mentioned possibility of adding production bonus)
• PTO : 4 weeks

• Relocation : $3,000
• CEs : Provide time off & financial contribution
• Licenses / Assoc / Fees : Eligible for loan forgiveness / repayment. Paid memberships to ADA & more. Covers cost for state license, DEA license & malpractice insurance
