Job Description
The Provider Support program provides community-based crisis prevention and intervention services for individuals with intellectual disabilities (ID) and mental health needs.
Our program works to enhance the ability of the system of care to successfully support individuals with ID and mental health needs in their community.
Using positive behavioral supports, multi-model clinical assessments, and other therapeutic tools, services are provided to caregivers supporting those with ID.
Provider Supports promotes systemic, person-centered approaches and provides training, education, consultation, crisis plan development, and crisis support.
This position requires regular travel throughout service area (mileage reimbursed) and rotational on-call responsibilities
Primary Duties and Responsibilities
Must be capable of regular and predictable attendance at a specific location to perform assigned tasks. Regular travel is required.
Maintain an active caseload of individuals referred for services (variability occurs depending on the activity level of cases and the project scope).
Ensure the coordination of support meetings and crisis plan development for individuals enrolled
Provide outreach visits to the individuals’ home, day, and vocational settings regularly
Use of clinical documents and other assessments for case planning and strategizing.
Develop and maintain clinical documentation and any other applicable documentation of services provided.
Develop materials and present community training as assigned by the director or designee, or when identified independently
Complete all required data entry expectations, including adherence to timelines established by the program
Provide on-call crisis response and assessment on a rotational basis
Renewal of certification every two years after initial completion
Participation in program specific meetings
Participate in the Center for START Services National Network, which brings together projects from across North America through an online resource center, conference calls, select study groups and a meeting at an annual conference as recommended
Other duties as assigned
Qualifications
A Master’s Degree in Social Work, Psychology, Counseling, or other human service field with 2 years’ experience working with people who have ID and mental health and / or challenging behavior needs.
A Registered Nurse or bachelor’s degree in Social Work, Psychology, Counseling, or other human service field with 4 years’ experience working with people who have ID and mental health and / or challenging behavior needs is acceptable.
