Family Coordinator /Mobile Crisis Responder

Position Summary The Family Coordinator/Mobile Crisis Responder performs general functions as a primary and secondary crisis worker in family/child/adults advocacy working within community .The Family Coordinator will provide primary and secondary crisis services for juveniles and their families, working to identify strengths, goals, while addressing potential barriers to success. The Family Coordinator will connect the family with behavioral and mental health resources to promote positive outcomes. As Mobile Crisis Responder is to assist clients in crisis in developing a stabilization plan they can follow when returning home after the crisis experience as well as helping identify how they can use coping skills to help in the next situation. Location may vary based on location of clients.

Job Descritption

· Complete outreach to promote programs to include a 8 county service area mobile crisis as well as a twenty-seven-county program providing support and advocacy to juveniles and placement families after a DHS coordinated removal of a juvenile from their home.

• Ensure appropriate delivery of services and coordination of care for client and families
• Provides excellent patient driven care and demonstrate superior positive regard to all clients
• Function in a multi-tasking, multiple priorities environment while maintaining accuracy and attention to detail
• Provide services to families and children, while timely updates of progress, barriers, and concerns to the referring agency and supervisor.
• Ensure contract deliverables are met and grant requirements are adhered to.
• Receive referrals for programs and ensure referrals are followed up on in a timely manner consistent with program requirements.
• Meet with families in their homes or other preferred location with appropriate regularity determined by program requirements and family needs.
• Complete required documentation and maintain accurate records within the Electronic Health Record.
• Maintain a caseload.
• Assign cases to other employees working cases within the scope of this program while providing supervision, support, and maintenance of all case records within the Electronic Health Record.
• Collaborate with supervisor(s) regarding intakes and case needs.
• Communicate with referring agency/worker, supervisor, and Elevate Team.
• Promotes integration of services for patients including behavioral health care and long-term services and supports to enhance the continuity of care for CCBHC patients.
• Remain available to clients via phone during working hours.
• Identifies critical incidents and develops crisis plans.
• Engage with clients that have cancelled or no-show to appointments to get them re connected to services.
• Coordinator for the clients and needed services/resources with both internal and external service providers.
• On call during working hours to respond within hour of call for those in need of Mobile Crisis Services.

Qualifications

• Crisis Intervention with experience in suicide intervention, and postvention
• Knowledge of community resources
• Experience assisting people with addiction, relationship difficulties, depression, stress, extreme poverty, mental illness, parenting etc.
