Elder Options providing care management & home care services in the Placerville area is looking to add an Associate Care Manager to our team. Minimum education is B.S. degree.

Must have ability to work with a high degree of autonomy & professionalism. Must have ability to work as a team under the direct supervision of the Clinical Manager.

Good computer & communication skills required.

Direct practice includes: Home visits, assessments, care coordination, accompaniment to medical appointments, identification and coordination of recreational therapy activities and applicable resources, crisis intervention, and creation and monitoring of person centered care plan. Extensive training is provided & included with this position.

Duties include:

Client/family home visits , person centered care plan development, communication with client/families, and medical professionals. Identifying unmet needs and understanding wishes & expectations of client/family. Oversite of complex medical issues/concerns, medication monitoring, accompaniment to medical visits. Knowledge in making referrals, understanding program eligibility, and coordinating care needs with community resources. Enhancing daily life with identification of activities/socialization, family meetings, and crisis intervention. Excellent documentation skills and ability to bill for one's time is required.

Assigned caseload after comprehensive training will be west slope of El Dorado County.

Pay Rate: $21.00/hour
