Job description

Title: Care Coordinator- Targeted Case Management

Supervisor: Targeted Case Management Supervisor

General Qualifications: Must possess a minimum of a bachelor’s degree in rehab counseling, psychology, social work, or other social services related field.

Position Description: This position will provide Case Management services to clients in care at the El Paso Child Guidance Center. Care Coordinator will engage with youth, adults, and families to empower them towards self-sufficiency. This position will provide weekly or biweekly sessions to children, adolescents, and their family members. Services will include CANS/ANSA assessments, treatment plans, skills training, and medication training and support conducted by evidence-based curriculums. Services will be conducted in a variety of settings to include office based, home based, community based, or telehealth based on client preference. Caseload size will be approximately 25 clients. This number can fluctuate based off the level of care of each client on each caseload and is an average number.

Classification: Hourly, non-exempt

Job Responsibilities: Complete onboarding processing, including training and certification in all evidence-based methodologies, not limited to but including Wraparound, when applicable.

  • Maintain caseload of TCM clients in care, completing also associated administrative and documentation task.
  • Conduct CANS/ANSA assessments with clients, ensuring timely completion of all assessment and intake paperwork.
  • Establish and maintain relationships with all youth and families throughout service delivery.
  • Ensure all documentation is completed and entered in all required databases in a timely and efficient manner.
  • Provide crisis intervention and safety planning for youth and their families as needed.
  • Actively participate with clients to determine best treatment for recovery. Work involves coordination and linkage activities to community-based services.
  • Maintain client confidentiality at all times and be familiar with all HIPPA laws and regulations
  • Participate in interdisciplinary case staffing meetings, trainings, and supervision as appropriate. Present on caseload to get feedback of course of client’s treatment.
  • Other duties as assigned by TCM Supervisor and/or Behavioral Health Director.
