Eklego is Hiring a Nurse Practitioner for Outpatient Primary Care Oncology Near Atlantic, NJ

Eklego is seeking a compassionate, and clinically astute Nurse Practitioner for Outpatient Primary Care Oncology to join a team of nursing professionals with the prestigious Mount Sinai in Atlantic City, NJ. Come be a part of an innovative team with Mount Sinai and the University of Pennsylvania Health system who have partnered with UHH to deliver a unique and innovative oncology program for UHH members.

Since 2016, UNITE HERE Health (UHH) and Mount Sinai Solutions have collaborated to enhance patient care by creating a medical neighborhood, which provides UHH members with enhanced access and exceptional care through a dedicated primary care center.

UHH is an innovative purchaser of health care, providing health care benefits for 14,000 members and their dependents who work in the entertainment and casino industry in Atlantic City. UHH has contracted with Mount Sinai Solutions since 2016 to operate a dedicated health center for its covered lives. The UHH Atlantic City Health Center is operated outside the pressures of a fee-for-service reimbursement model and instead is focused on access and outcomes.

Mount Sinai Solutions is part of the Mount Sinai Health System focusing on how to provide population health solutions direct to employers and purchasers such as union health funds. Mount Sinai Solutions operates six dedicated health centers and two shared health centers for employers and purchasers in New York and New Jersey.

What will I do?

This role is a unique opportunity to incorporate clinical skills providing primary care services for our patients undergoing active cancer treatment, program development and care coordination.

CLINICAL PRACTICE

  • Provide primary care services with a focus on the subset of the population undergoing active oncology treatment. Care includes triage, initial evaluations, care planning and primary care follow-ups
  • Collaborate with Penn Oncology Team and Mount Sinai Medical Director on patient care, treatment of chronic conditions and/or concerns arising during oncology treatment.
  • Initiates referrals to specialty services and fosters continuity of care between Penn Oncology and Mount Sinai.
  • Works with Mount Sinai internal Social Worker to identify additional resources for patients in active cancer treatments as well as cancer survivors
  • Maintains patient/employee confidentiality in the management of information.
  • Develops the plan of care based on the diagnoses and individual patient needs, records, reports, and interprets the clients' responses to the plan of care.
  • Performs a patient record review with the Mount Sinai Medical Director and Penn Oncology Team weekly or as needed.
  • Leads regular patient coordination meetings with Penn and UHH

  • Participates in defining, maintaining, and interpreting standards of nursing practice.
  • Contributes to the maintenance of a positive image of advanced practice nursing in the medical center and in the local community by implementing programs which recognize nursing as a profession.
  • Participates in departmental and interdisciplinary committees which influence and/or determine policies affecting nursing practice and patient care delivery.
  • Seeks opportunities to enhance professional nursing practice and influences outcomes.
  • Treats co-workers, patients and families with dignity and respect.
  • Is open and responsive to the diverse backgrounds and experience of other people, and promotes an environment that is sensitive to cultural diversity.

KNOWLEDGE, SKILLS AND ABILITIES REQUIRED

  • Ability to perform history taking and physical assessment techniques.
  • Demonstrate and complete documentation of competency in specialty specific procedures according to established standards as part of initial orientation and annual performance review with collaborating physician, oncology team and case managers.
  • Ability to interpret and apply principles of nursing research utilization.
  • Ability to manage and coordinate a patient caseload.
  • Work within a Collaborative Practice Agreement or Collaborative Practice Relationship Attestation as a member of the health care team in area of practice
  • Ability to work collaboratively across organization to develop new program

Requires:

  • Licensure: New Jersey State License with current registration as a registered nurse.
  • State Certification: Certified by the New Jersey State Department of Education as a Nurse Practitioner with current registration as follows:

Education:

Master's degree program designed to prepare a NP to diagnose, treat, and prescribe for a patients condition that fall within their specialty area of practice or the successful completion of an approved supplemental education program, i.e., post-master's NP certificate.

Experience:

Previous Primary Care and Oncology experience

For all NPs hired effective June 1, 2019, National Board Certification in Advanced Practice Nursing required. For NPs who bill for professional services, National Board Certification is required regardless of hire date. The Board Certification requirement may be deferred up to 1 year if approved by Chief Nursing Officer or designee.

Credentialed through the Credentialing Committee of the Medical Board with approved Delineation of Privileges

Drug Enforcement Administration (DEA) certificate and Basic Life Support (BLS) certification required

Advanced Cardiac Life Support (ACLS)
