Eightfold is Hiring a Speech Language Pathologist Pediatrics Near Rochester, NY

The therapist is responsible for the evaluation, diagnosis, treatment, and recommendation of services for enrollees. The therapist promotes optimal function and engagement in meaningful activities; provides support to families and collaborates with other disciplines as needed in order to address goals and objectives as indicated in each enrollee's IFSP or IEP. In compliance with state and local requirements, each therapist will be responsible for maintaining appropriate documentation regarding enrollee services and subsequent needs.

ESSENTIAL FUNCTIONS

· Evaluates, diagnoses, and manages enrollee impairments and disabilities.

· Provides therapy services to enrollees and initiates referrals when appropriate and in accordance with agency procedures.

· Conducts screenings of enrollees at regular intervals to determine need for interventions/treatment.

· Evaluates enrollees to obtain data necessary for treatment planning and implementation. Conducts specialized evaluations as indicated.

· Obtains appropriate physician’s orders/documentation for treatment.

· Interprets and communicates evaluation findings to enrollees, families, and other team members.

· Develops treatment plan by identifying a target list, including short- and long-term goals and methods to achieve identified goals.

· Coordinates, implements, and conducts treatment and plans with enrollees, family, and other team members.

· Assesses the need and makes recommendations for assistive devices, orthotics and positioning, and follows up as needed.

· Monitors enrollee’s response to intervention and modifies treatment as indicated to attain goals in accordance with plan and physician orders.

· Develops appropriate home therapies or programming within the community to maintain and enhance the performance of the enrollee in their environment.

· Documents all results of enrollee’s assessment, treatment, follow-up, and termination of services.

· Adapts treatment to meet the individual needs of the enrollee.

· Reviews the quality and appropriateness of the total services delivered and individual therapy programs for effectiveness and efficiency, using pre-determined criteria.

· Provides in-service education to the members of the enrollee’s care team.

OTHER DUTIES

· Adheres to Daystar’s customer service principles and serves as an ambassador for the program.

· Attends meetings, program events and professional development activities, as required.

· Accomplishes all other duties and tasks as appropriately assigned or requested.

EDUCATION & EXPERIENCE

· Master’s Degree in designated therapy specialty

· Must hold current license in New York State

· Minimum 3 years’ experience

KNOWLEDGE, SKILLS & ABILITIES

· Ability to communicate clearly and professionally, both verbally and in writing.

· Understanding of effective teaching methodologies and tools.

· Demonstrated ability to work collaboratively with team members.

· Excellent public speaking and presentation skills.

· Proficient in Microsoft Office suite.
