Case management is a collaborative and rewarding process as you help others navigate through the complex and often overwhelming healthcare system; it can be a source of great personal satisfaction. We are seeking a compassionate RN, with top-notch communication, problem solving and time management skills for a full-time, direct hire opening!

In this role, you will manage a caseload of approximately 25-30 members, monitoring the health and well-being of the pregnant woman and their child after birth. You will provide an interactive parent-child education curriculum that focuses on helping parents to develop skills in emotionally connecting with their child and use motivational interviewing to help clients clarify their goals, develop problem-solving skills, and move toward increased self-sufficiency. You will visit clients on a weekly or bi-weekly basis, beginning early in pregnancy and continuing until the child is two years old, allowing you the opportunity to form deep bonds with your patients.

Days/Hours: This position is full time, days, Monday - Friday, no weekends and no major holidays.

Salary: $55,341-$77,584/year

Job Requirements

Minimum Qualifications:

• A valid NYS RN license in good standing is required
• BSN is highly preferred
• Prior case management, pediatric or Mother-Baby experience is preferred, but not required
