REGISTERED NURSE

RESPONSIBLE TO: Nurse Manager/Residential Services

FUNCTION: To serve as an active member of the Interdisciplinary Team; To promote and enhance person-centered outcomes; To serve as medical coordinator/advisor/liaison on behalf of consumers medical needs/interests in conjunction with family member wishes and community provider recommendations; To ensure consistent compliance with established treatment plans; To serve as a mentor to LPNs and recent graduates; To promote a life of hope, dignity, and purposeful living for all.

SPECIFIC DUTIES:

1. Monitors the provision of routine and specialized medical care and ensures proper follow-up on all recommendations from all providers, family members, and advocates. Routinely consults with associated providers/advocates, as appropriate, to ensure all medical needs are met, consistent with established treatment plans.

2. Monitors the provision of psychiatric services and provides accurate input from the team, as applicable.

3. Monitors all health-related documentation and ensures that medical records are complete, accurate, and up-to-date. Advises team of any significant medical changes in a timely manner.

4. Provides on-going and timely in-service trainings to all staff, as applicable (i.e. changes in care plans, procedures, practices, etc.).

5. Actively participates in and offer guidance/support to ISP, IDT, and staff meetings.

6. Promotes universal precautions and assists in the control of communicable/infectious diseases, under the direction of the RN Manager.

7. Actively assists in the admission process (i.e. screening/evaluation of applicants for proper placement, and identifies/ensures any specialized healthcare needs can be met).

8. Maintains adequate, up-to-date, and readily accessible supply of medical supplies and equipment for daily use as well as in the event of an emergency, as outlined in the Disaster Plan.

9. Performs all other duties as needed or assigned.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES: Must possess strong nursing skills and abilities in order to provide comprehensive care on a daily basis as well as in emergency situations; A firm understanding of current and relevant nursing/medical policies/procedures/practices is essential, Must possess extensive knowledge of all applicable state and federal healthcare regulations (i.e. HIPPA, etc.); Comprehensive understanding of person-centered planning and associated principles/methodologies is critical; Must possess strong organizational and time management skills; Must be self-directed and able to initiate and manage multiple responsibilities effectively and efficiently; Excellent verbal and written communication skills across various channels is paramount; Experience with individuals with disabilities is helpful; Ability and willingness to learn new technology (i.e. EMR, etc.) is important; Must be flexible and willing to work non-traditional hours as needed or assigned to meet consumer need (including participation in on-call status on a rotating basis); Physical condition commensurate with the demands of the job; Ability to lift a minimum of 50 pounds waist high is required.

MINIMUM QUALIFICATIONS: Bachelors Degree in Nursing, or related field, with New York State licensure to practice as a Registered Nurse (graduates must obtain licensure within 90-days of offer and must work under the direct supervision of licensed RN Supervisor or RN Manager), plus one (1) year of experience serving in a nursing capacity (excluding graduates); OR Associates Degree in Nursing, or related field, with New York State licensure to practice as a Registered Nurse (graduates must obtain licensure within 90-days of offer and must work under the direct supervision of licensed RN Supervisor or RN Manager), plus three (3) years of experience serving in a nursing capacity (excluding graduates). Professional experience in skilled nursing facility strongly preferred. Valid driver’s license, acceptable driving record as established by the agency’s insurance carrier, use of a personal vehicle, and willingness to travel is required. This is a position that is exempt from overtime provisions of the Fair Labor Standards Act.
