Schedule: On Call Monday-Friday Rotation

Description:

•  Regularly re-evaluates patient nursing needs.
•  Initiates the plan of care and makes necessary revisions as patient status and needs change.
•  Uses health assessment data to determine nursing diagnosis.
•  Provides health care instructions to the patient as appropriate per assessment and plan of care.
•  Visit clients routinely in order to observe and evaluate each clients physical and emotional status.
•  Maintain the confidentiality of all client care information.
•  Monitor nursing care to ensure that all clients are treated fairly, and with kindness, dignity, and respect.

Qualifications

•  At least 1 year experience in Home Care preferred
• 1-2 years RN experience in acute care
•  Valid RN License
