Offers are made at the entry level of the range. Salary placement will be determined based on experience, education and licenses held.Assignment:The position will be full-time, working 200 days per year. Frequent travel throughout Clark County will be required to fulfill the functions of this role and reimbursed in accordance with ESD 112 travel policies and procedures. We have three vacancies available. Essential Functions/Other FunctionsEssential Functions:
Provide assessment, intake, case management and therapeutic support to identified youth at school, within the home, or while placed within community programs.
Work with students and their guardians in the development of an individual support plan which is strength based, measurable, and assists students in meeting their goals.
Assist with implementation and monitoring of supervision and support plans as needed.
Provide individual, family and group counseling.
Meet with students, families, counselors, school officials, juvenile justice, and other community partners to facilitate the student's school enrollment, job placement or other identified supports.
Coordinate and participate in a multidisciplinary team including but not limited to guardians and other natural supports, school and district staff, juvenile justice, primary care, community partners and other team members as indicated.
Identify barriers to youth success in the school and community. Refer and connect youth/students to appropriate community resources and services (i.e., support groups, mentoring, tutoring, clothing, health care, food, housing, transportation).
Monitor progress and attendance at school. Assist youth and family with tracking and organization of homework assignments; assist students with scheduling classes and credits needed to stay on track to on-time graduation or for recovering lost credits.
Administer and assess evidence-based measurement tools to track student improvement, success and readiness for service conclusion.
Maintain complete and timely documentation as required by law and administrative regulations.
Other Functions:
Participate in staff and team meetings, in-service programs, workshops and seminars.
Maintain knowledge of best practices in the mental health field.
Follow policies and procedures for ESD 112 and the Behavioral Health Agency.
