EDMOND HEALTH CARE CENTER is Hiring a Social Services Director - Long Term Care Near Edmond, OK

Voyage Long Term Care delivers the highest quality of care for every resident through our most valuable asset: you! We are looking for a compassionate individual with experience & interest in serving the social service needs of our residents.

Essential Job Functions:

  • Assure that social service/discharge planning activities are integrated and coordinated within the facility team.
  • Develop specific, measurable, realistic plans and objectives, which enhance social service daily.
  • Perform psychosocial assessment and reassessment on all residents. Identify and address the psychosocial needs of residents and their families.
  • Demonstrate knowledge of clinical social work and discharge planning. Interpret and promote residents’ rights.
  • Interact professionally with resident and family and involve resident and family in the formation of the plan of care.
  • Provide individual and group help for residents’ and families at times of adjustment, crisis, or need.
  • Coordinate and direct Social Service/Discharge Planning Department to ensure resident needs are met.
  • Provide appropriate resources and support to Social Service/Discharge Planning Staff/Clinical Staff.
  • Be flexible, organized, and function under stressful situations.
  • Maintain a good rapport and working relationship with medical staff, departments, and employees.
  • Manage and operate equipment safely and correctly.
  • Treat patients and their families with respect and dignity.
  • Support and maintain a culture of safety and quality.

Qualifications:

  • Experience in long term care is certainly preferred, though not required.
  • Candidates must maintain a working email address and phone number for employer communication.
  • Ability to read, write and speak the English language
  • Ability to work cooperatively as a member of the team
  • Ability to communicate effectively with residents and their family members, and at all levels of the organization
  • Ability to react decisively and quickly in emergency situations
  • Ability to maintain confidentiality
  • Knowledge of training techniques for clinical staff
  • Good ethical practice
  • Relationship management
