• Title: Care Coordinator (case manager) nurse positions within our Primary Care clinic:
• Requirement:
• Shift: Weekdays Monday through Fridays from 8a to 5p, with One (1) hour lunch, no weekends or holidays. Collaborate with support nursing staff and with providers for patient care.
• Housing: Information can be found under the section.
• Orientation: on Mondays only - If your traveler misses the compliance deadline, they will be pushed to the following scheduled Orientation.
• Guaranteed Hours:36
• Pay Rate: $63/hr
• Gross Rate: $2268/Wk

Pay: $60.00 - $63.00 per hour

Expected hours: 36 per week

Experience:

• Nursing: 1 year (Preferred)

License/Certification:
