JOB SCOPE : Responsible for coordinating the Clinic intake flow, obtaining referral information and documentation, conducting intake assessments and determining client needs and initial treatment recommendations.
Responsible for requesting and reviewing documentation provided by client and outside parties and making sure collateral information is integrated in case conceptualization.
Responsible for intake registration, assignment, coordination of services with other team members and follow up. In addition, assesses and intake of the client to determine eligibility for services in the clinic.
Makes sure new intake client will be assigned to therapist. ESSENTIAL FUNCTIONS : Responsibilities include but are not limited to the following : Conducts Intake interviews and assessments.
Conducts telephone screenings where necessary.Carries a case load as needed.Complete as many intakes as needed per week.Develop and maintain referral resource database.
Presents results of interviews and assessments at supervision or staff meetings.Consults with Clinic management and other staff as necessary.
Maintains all required documentation and records on a current and timely basis.Participate in disposition meetings as required.
Performs other duties as may be assigned by management.Represents the company and Clinic as needed.Salary : $58,000 to $60,000 annually EDUCATION AND EXPERIENCE : LMSW, LMHC or LCAT required.
NY State license required.Experience working with individual clients and families in a mental health setting.Experience conducting intake / psychosocial assessments.
KNOWLEDGE : Psychotherapy training and experience. Excellent clinical and administrative skills. Ability to stand long period of times.
Computer literate, Word, Outlook, Excel. EMR preferred. OMH Article 31 guidelines SKILLS AND ABILITIES : Excellent oral and written communication skills.
Excellent organizational and planning skills.Demonstrated leadership qualities and ability to interface with other employees.
Demonstrated ability to exercise sound judgment.Ability to work proactively, problem solve, thinking critically, and strategically.
Ability to handle multiple tasks and shift between responsibilities.Ability to work effectively with others and demonstrate conflict resolution skills.
