Job Title: Healthcare for the Homeless Staff Nurse

Position Summary:

The LPN will be clinically competent while delivering care to patients utilizing the nursing process of assessment, planning, intervention, implementation and evaluation. The LPN will effectively interact with patients, significant others and other health team members while maintaining all standards of professional nursing.

Principal Responsibilities:

  • Prepare patients for exam and treatment to include: taking a history, vital signs, all measurements (weight, height, head circumference, girth, etc.), and hearing and vision screens.
  • Collect and document pertinent patient medical information as outlined by organizational guidelines consistent with nationally recognized quality standards. (UDS, HEDIS, Meaningful Use, & PCMH).
  • Coordinate patient care utilizing available resources to include Best Chance Network, Patient Assistance Program, Smoking Cessation, HIV opt-out testing, etc.
  • Carry out provider orders that are within the scope of practice as outlined by the SC State Board of Nursing utilizing proper protocol established by the organization, & healthcare regulatory agencies.
  • Follow organization policies & procedures regarding proper collection, handling and electronic submission of patient specimens.
  • Follow appropriate protocol for performance of CLIA waived testing.
  • Perform telephone and walk-in triage according to approved protocols.
  • Assist providers with medical procedures and examinations as needed.
  • Assist medical providers with prescription management as appropriate based on organization protocol.
  • Patient education and distribution of preventative health material.
  • Read, record, analyze, and distribute lab results, hospital reports, and data relating to patient care.
  • Assist with monitoring and ordering medical equipment, immunizations, in-house and sample medications.
  • Comply with regulations set forth by DHEC and all other regulatory agencies to ensure compliance of funded vaccine programs for children and adults.
  • Abide by pharmacy policies & procedures in compliance with the SC Board of Pharmacy regarding medication storage, destruction of expired medications, maintenance of sample and in-house medications, oxygen storage, etc.
  • Assist with preparation, stocking, and cleaning of patient exam rooms and lab area as needed.
  • Provide services in various homeless shelters and mobile clinics across the midlands.
  • Other duties as assigned.

Education & Experience :

  • Graduate of an accredited school of nursing. Currently licensed to practice as a LPN in the state of South Carolina.
  • Current BLS certification.
  • Two years’ experience in a clinical patient care setting preferred.
  • Reliable transportation required.

Physical Demands

  • Prolonged periods sitting or standing
  • Must be able to lift up to 25 pounds.
