Who We Need:

We’re looking for an in-home Nurse Practitioner to join our Clinical team. This role will report to our Chief Medical Officer and Director of Nursing.

EasyHealth Medical Solutions completes Annual Wellness Visits for Medicare members/patients of primary care physicians, medical groups, and health plans we serve. The Nurse Practitioner will perform comprehensive assessments in home setting and / or in clinic setting (depends on need).
**Only For CA Licensed Nurse Practitioners (NP)**

Must be willing to travel in the San Diego area.

Bilingual Spanish (Preferred)

Status: Full Time

Schedule: Monday Through Friday (open to working on Saturdays if needed)

Pay: $130,000 per year plus incentives

What You’ll Do:

• Clinical Functions
  • Complete a detailed evaluation of the medical and social assessment of the beneficiary members using Easy Health documentation tool or client EMR.
  • Identify and refer members/patients for routine, urgent, and emergent case management.
  • Make recommendations for member’s follow-up with their primary care physician.
  • Not required to prescribe or alter the members current treatment plans.
• Stewardship - confirm beneficiary information and conduct clinic visit as per assignment.
• Maintain flexibility in schedule and is available to assist with unexpected emergencies.
• Privacy - maintain comprehensive understanding related to protected health information and observe HIPAA requirements.
• Professionalism - maintain warm, caring, and courteous behavior with the members.
• Performance - complete a comprehensive exam. All diagnoses listed in the documentation must be supported. Discuss care guidelines with the member. Escalate emergent and urgent condition appropriately as per the guidelines.
• Member Satisfaction - make sure that the member is satisfied with the exam and overall visit.
• Member Communication- maintain member privacy and courteous communication with members and colleagues.
• Safety - maintain safe working environment. Follow safety policy and procedures listed subsequently.

Who You Are:

• 2 years hands on experience as a Nurse Practitioner.
• Current, unrestricted license in the state or the ability to acquire one.

• Board certified, preferably geriatric, or adult medicine specialty.
• Familiarity with HCC model and quality HEDIS and other STAR measures
• Effectively communicate with elderly and chronically ill members
• Proficient in use of computer, tablet, and mobile technology
• Bilingual Spanish / English (Preferred)
• Must be willing to travel.
