Rate: $22.47 per hour
Schedule: M-F 8a-5p varies
Site: Coldwater Road, varies
Incumbent performs duties according to standard Division policies, procedures, goals and objectives, exercising discretion and independent judgment in effectively applying guidelines to individual cases and circumstances and in supervising assigned personnel. Incumbent makes a substantial contribution to the agency's mission by directing effective training programs according to agency policies and procedures. Incumbent works with minimum direct supervision, referring to supervisor any unusual or unprecedented situations, such as misconduct of agency staff. Work is periodically reviewed for compliance with agency policy and effect on agency goals and objectives. Incumbent maintains frequent contact with other Easterseals Arc personnel, other agencies, government officials, the public, clients and their families/guardians for the purpose of exchanging information, coordinating, directing and supervising training operations to meet client needs.
Duties and Responsibilities:
Maintain compliance with all regulatory bodies
Trains and assists direct support professionals in preparing for local, state and federal reviews
Ensures that services adhere to regulations.
Maintains detailed client case notes, compiles and submits reports.
Is accountable for client finances. Accurately tracks, monitors and documents all approved billing
Ensures accurate and proper documentation of services; ensures that services adhere to regulations
Ensure services provided are individualized
Schedules and facilitates team meetings to assists in the development of the individual support plan (ISP) and person centered plan (PCP)
Monitors for progress and ensures follow through with objectives listed in the individual's ISP and BSP
Applies for and refers to community resources to meet the emotional, financial, and vocational and education, legal and social needs of the individual
Provides crisis intervention/supportive counseling, assists with supervision of clients as needed, participates in on-call rotation
Completes weekly site visits to ensure that all the individuals' plans are implemented as written, documentation in the home is correct and to ensure that the individual's needs are being met.
Develop a workforce that fosters working through a continuum of services
Supervises assigned personnel, including interviewing, hiring, orienting, training, disciplining and terminating, reviewing and approving work schedules. Shall perform duties of absent personnel as needed.
Adheres to Easterseals Arc Human Resources and EEO/AA policies and performs other duties as assigned
Provides staff training on each participant's current ISP, BSP and high risk plans. Ensures that all staff are properly trained prior to working with participants and that the training is documented according to department policy.
Support people to be safe and have optimal health and well being
Coordinates medical services for individuals including hospice, hospitalizations, rehabilitation and routine medical services, providing transportation as needed
Arranges for special medical services, including hearing aids, therapies and orthopedic shoes, adaptive equipment etc.
Ensures ordering of medication refills as needed
Monitors client MAR's, ensure MAR documentation is correct and complete
Positively represent Easterseals Arc.
Uphold agency values and follow agency policies and procedures
Maintain confidentiality of records/information according to HIPAA, State, Federal Laws and guidelines
Maximize and responsibly utilize agency and community resources
Serves as liaison to participants, families and community resources
Serves as liaison to State and local agencies
Requirements:Qualifications:
At least 18 years of age
High School diploma or GED required.
Experience working with individuals with autism and other developmental disabilities; prefer 2 years.
1 year of supervisory experience preferred.
Ability to pass criminal/registry background checks and other checks deemed necessary
A valid driver's license & insured vehicle
Basic language, written and computer skills necessary for communication and documentation
Ability and willingness to support individuals with challenging behavior and/or medical concerns
Ability to pass and maintain required trainings such as CPR/First Aid, Medication Administration and Positive Behavioral Support Training and other trainings as deemed necessary by the agency
Working Conditions: QIDP's work in a changing environment that adjusts to the needs of the individuals in program services. The environment may be fast-paced with interruptions, distractions and deadlines. QIDP's need to be flexible and be able to adjust to changing needs, activities and locations. Some sites may have tobacco smoke and/or pet dander based on the individual being supported. QIDP's must have an insured, safe vehicle in good working order to utilize for incidental transportation required for staff and program participants to and from community activities. Incumbent may work with violent/physically aggressive participants and responds to emergency situations involving potential physical harm to self and others. Incumbent may work extended hours, evenings, and occasionally travels out-of-town as needed.
PHYSICAL REQUIREMENT- Depending on the needs of the individuals, the work can be physically demanding including transferring or supporting people with mobility needs, lifting or moving items up to 75 lbs, walking or standing for extended periods of time or providing support through CPR/First Aid, behavioral support, and other interventions
