Easterseals of Mahoning, Trumbull and Columbiana Counties is looking for an energetic, compassionate team player to serve our clients and help make a difference. We are currently looking for a full-time Activity Coordinator. This position will assist clients in the Adult Day Center in Mahoning and Columbiana Counties. The right candidate for this position will be required but not limited to:

JOB OBJECTIVE:

Administer, coordinate, and implement the Home and Community-Based Service programs in accordance with professional and ethical guidelines to ensure that activities meet the individual participant's needs, and interests and maintain or enhance the individual’s quality of life.

ESSENTIAL DUTIES:

· Plan, schedule, and conduct social, physical, and educational activities that stimulate and maintain the participant's abilities as determined by the individual plan of care for Easterseals locations within Mahoning and Columbiana Counties.

· Plan and post the activity schedule and/or event every month and ensure implementation on a group and/or individualized basis as indicated by the participant's care plans.

· Inventory and process activity supply needs in accordance with established procedures to ensure the availability of resources needed to maintain ongoing care of participants.

· Implement effective communication techniques and provide direction to the client care staff to ensure continuity of care for all participants.

· Adapt activities to service all adults in Home and Community-Based Programs.

· Provide input into the review of program data, processes, productivity, and/or financial status to ensure efficient and effective management of human, fiscal, and material resources.

· Evaluate and participate in multi-disciplinary team meetings with clients and their family supports, case managers, physicians, etc… based on the timelines regulated by the funding source.

· Track and chart client interactions and provide measurable outcomes of all program activities.

· Maintain effective communication with community agencies, organizations, and referral sources to facilitate client service and to provide education/support to the client/family.

· Maintain and submit all required credentials/licenses necessary to the position as received, updated, and/or renewed.

QUALIFICATIONS:

· Bachelor's or associate degree in therapeutic recreation or related field, two years of experience in a related position or NCCAP certification; knowledge of the population served by Home and Community Based Services.

· Ability to complete an initial activity assessment on newly admitted participants and develop an individualized plan that is related to the participant’s psychosocial needs.

· Ability to maintain daily records for the participant which indicate attendance and participation in activities.

· Ability to relate to the participants enthusiastically and appropriately to ensure maximal participation in each activity.

· Ability to assist with follow-up on concerns regarding the needs of the participant including financial, interfamily concerns, and personal needs.

· Ability to interact well with participants, families, significant others, outside personnel, co-workers, and other disciplines involved with the participant’s care.

· Ability to assist with the recruitment, training and supervision of department staff and volunteers.

· Utilize proper techniques used to assist clients with activities of daily living such as ambulation, eating, toileting, and self-direction.

· Ability to inventory and order equipment and supplies for use.

· Ability to safeguard, motivate, and assist participants in performing exercises and functional activities under the direction of the professional staff.

· Knowledge of behaviors associated with Alzheimer’s Dementia, Autism, ADHD, and related disorders and displays appropriate techniques used to prevent injury of self, participants, or others.

· Knowledge of abuse regulations and following agency policies and procedures when any incidents occur.

· Ability to appropriately perform cleaning, disinfection and maintenance of areas and equipment.

· Ability to identify the need for personal protective equipment and demonstrate the ability to properly utilize personal protective equipment as required in the performance of duties.

· Knowledge of and can abide by agency safety and infection control practices during the performance of duties.

· Ability to remain current in skills through attendance at in-service training, continuing education classes, and certification courses.

EQUIPMENT USED/MACHINERY REQUIRED TO OPERATE:

· General office equipment including computers and applicable software programs.

PHYSICAL DEMANDS

· Frequent standing, walking, bending, stooping, and twisting.

· Frequent pushing/pulling/maneuvering of mobility devices such as wheelchairs.

· Must be capable of safely lifting a minimum of 30 lbs. without difficulty.

FLSA Status – Non-Exempt/$16.00 per hour and $2,000 SIGN ON BONUS!

Classification – Full-Time – 40 hours per week

Benefits – PTO, ILL Time, Paid Holidays, Medical and Voluntary Benefits, 401(k), Flexible Schedule. No evenings or weekends.
