Job Responsibilities:

  1. Demonstrate the ability to engage individuals in a welcoming, hopeful, empathic manner regardless of disability or phase of recovery.
  2. Meet with new outpatient families in the community within 3 days or during the intake to introduce services and supports available. Assist with completing crisis plan during this time as applicable.
  3. Screen for co-occurring disorders and recognize diagnostic criteria used to identify substance abuse or dependency.
  4. Coordinates with Primary Behavioral Health Clinician in identifying needs, developing goals and creating authorizations for services. Reviews IPOS with this clinician to ensure needs are being addressed.
  5. Completes referral forms, clinical and legal documentation necessary to obtain community-based services for clients as well as collaboration and communication with other SA/MH community partners.
  6. Advocates and provides options for supports and services as needed and seeks out and develops community resources to meet the needs of assigned individuals for activities and support services.
  7. Educates individuals and provides resources for Self Determination options.
  8. Assist individuals with identifying changes in behavior, attitude, feelings and thoughts that are triggers for relapse. Assist in the development of a relapse prevention plan and crisis plan utilizing a variety of supports including personal, community and treatment interventions.
  9. Provide education regarding prescribed medications to treat substance use and psychiatric disorders.
  10. Participates in individual supervision and team meetings to review progress, identify barriers and develop strategies for better assisting families in reaching their goals. 
  11. For individuals hospitalized for psychiatric reasons, case manager will meet with the individual within 72 hours of the psychiatric admit and coordinate with all parties involved.
  12. For individuals discharged from the hospital, case manager will meet with the individual/family within 72 hours in the office or community. and coordinate with all parties involved.
  13. Crisis plan will be updated/reviewed within 30 days of hospital admit.
  14. For individuals utilizing crisis intervention, case manager will assist primary clinician with engaging family and reviewing/updating crisis plan as needed within 72 hours of incident.
  15. Continue to meet with client/family a minimum of 1x per month for 90 days if psychiatric hospitalization or crisis intervention incident occurs.
  16. Meet with clients weekly that are residing at Children’s Village.
  17. Ensures that paperwork is maintained in accordance with funder requirements as well as Easterseals policies and procedures.
  18. Travels extensively to various locations throughout Oakland County to assess and evaluate the needs of clients and develop program goals.
  19. Demonstrates knowledge of principles of trauma informed care and co-occurring disorders.
  20. Willingness to support and assist consumers with trauma related issues.

Minimum Qualifictions:

  • Must be a CMHP in accordance with Medicaid Provider Manual Guidelines. Must meet one of the following:
    • Possess a Bachelor's degree from an accredited school is trained and has three years supervised experience in the examination, evaluation, and treatment of minors and their families.
    • Possess a Bachelor's degree from an accredited school and be an LLBSW or LBSW with the State of Michigan and one year of experience in the examination, evaluation, and treatment of minors and their families.
    • Be a Master’s prepared LLMSW or LMSW with the State of Michigan with specialized training and one year of experience in the examination, evaluation, and treatment of minors and their families.
    • Be a Master’s prepared LLPC or LPC with the State of Michigan with specialized training and one year of experience in the examination, evaluation, and treatment of minors and their families.
    • Be a Master’s prepared TLLP or LLP with the State of Michigan with specialized training and one year of experience in the examination, evaluation, and treatment of minors and their families.
  • Must possess a valid motor vehicle operator's or chauffeur's license.
  • Must possess a knowledge and understanding of co-occurring disorders (mental health & substance abuse); Stages of Change (The Transtheoretical Model); Phases of Recovery, and Motivational Interviewing.
  • Ability to work independently as well as in a team environment.
