Job Responsibilites:

  1. Provides individual in home assessments, individual counseling services based on a strength-based model and develops individualized strength-based treatment plans.
  2. Engages consumer and family in the treatment process and goal-setting. Identifies risk factors including lethality and responds to barriers to successful treatment.
  3. Works constructively with consumer to reach agreed upon outcome, and coordinates care with internal and external providers.
  4. Provides non-emergent in home assessments and psychotherapy for Senior Reach referrals. This is to be accomplished by working as a team member with Senior Reach staff.
  5. Maintains appropriate professional standards and provides appropriate follow-up for clients.
  6. Maintains up-to-date knowledge of community agencies, resources and other services for older adults.
  7. Provide proactive outreach including presentations on issues that are key to the aging population, their care-givers,
    and professional organizations and non-traditional organizations that serve that population.
  8. Responsible for accurate and timely data collection, documentation and reporting in accordance with Senior Reach
    standards of practice and any funding requirements.
  9. Attends mandatory in-services, on-line training and any individualized training plan as required.
  10. Participates in supervision by coming prepared with an agenda. Reports high risk/problem cases, and utilizes a
    problem solving approach.
  11. Attends supervision at times and intervals agreed upon with supervisor.
    Meet required number of consumer service hours as determined by Network Director.
  12. Be a team player and exhibits enthusiasm, courtesy, adaptability, flexibility, and spirit of cooperation in the work
    environment.
  13. Maintains effective interpersonal relations with consumers, peers, subordinates, upper management, visitors and the
    general public.
  14. Effectively responds to client needs and problems, initiates and maintains positive interactions, timely response to
    phone calls, pages, email and other requests.
  15. Works cooperatively with other community agencies, as appropriate, and in agreement with supervisor. Participates in related committees and/or task forces as needed.
  16. Flexible work hours may be required occasionally.
  17. Recruit/engage members for the Senior Outreach Advisory Group and facilitate the quarterly meeting

Minimum Qualifications:

  • Possess a Master's degree from an accredited college or university with a major in Social Work; AND Have a license as a Licensed Master Social Worker (LMSW)from the State of Michigan Department of Consumer and Industry Services.
  • Have two or more year’s related experience and/or training with older adults or gerontology certification.
  • Must meet qualifications as a Mental Health Professional (MHP) per the Michigan PIHP/CMHSP Provider
  • Qualifications per Medicaid Services.
  • Must have strong knowledge and ability to work effectively with the brief solution-focused treatment model; clinical
    practice with older adults preferred.
  • Must demonstrate ability to maintain a high level of efficiency and compliance with administrative rules and standards.
  • Possess strong knowledge base of service community, agencies and providers who work with older adults.
  • Possess a valid motor vehicle operator or chauffeur's license.
  • Position requires experience with relevant computer applications including electronic medical records.
