Easterseals Central Texas began in 1937 as the Texas Society for Crippled Children. In 1949, a small building was dedicated as the Austin Cerebral Palsy Center serving a small population of Central Texas residents. Almost twenty years later, we became the Capital Area Rehabilitation Center, expanding our services and those served. THE VITAL CASE MANAGEMENT ROLE YOU HAVE

n

As a CLASS Case Manager II, your daily mission revolves around providing unwavering support to individuals in the Community Living Assistance Support Services (CLASS) Medicaid Waiver program. Collaborating closely with the CLASS Administrative Assistant and seeking guidance when needed from the Senior Director, your commitment to consistent and quality care for clients is paramount.

Your role encompasses efficient case management, meticulous documentation, and fostering the principles of independent living, empowerment, and community integration. You empower clients to make informed decisions about housing, employment, education, and personal goals, maintaining close personal knowledge of their aspirations and challenges. Regular meetings to discuss progress and address issues align with each client's Individual Services Plan (ISP). Additionally, you are responsible for accurate program reporting and billing forms and staying updated on community resources and state/federal programs that benefit CLASS clients. Your role is a commitment to enriching lives, promoting independence, and fostering community integration!

QUALIFICATIONS FOR OUR CLASS CASE MANAGER II

    n
  • Master Social Worker license in the state of Texas, OR a master's degree in human services, drug and alcohol education, counseling, psychology, or criminal justice, OR a bachelor's degree in human behavioral science (30 semester or 45 quarter hours either in the development of human behavior, child development, family intervention techniques, diagnostic measures, or therapeutic techniques
  • n
  • 2 years of CLASS case management experience
  • n
  • 2 years of experience providing human services to children or adults with disabilities
  • n
  • Knowledge of Microsoft Office products or the ability to learn them
  • n
  • Strong communication skills
  • n
  • Ability to work independently and with minimal supervision
  • n
  • Highly analytical with extensive critical thinking skills for creating client plans
  • n
  • Basic mathematical abilities
  • n
  • Ability to travel throughout the North Texas area
  • n
  • Valid driver's license and proof of insurance
  • n

Spanish/English proficiency is preferred but not required.
