Summary: Oversees all aspects of CES Waiver referrals and intake by performing the following duties:
Essential Duties and Responsibilities: include the following. Other duties may be assigned.
Initial contact with referrals to determine service needs, answer questions and to begin intake process.
Coordinates with PASSEs and individual served or parent/guardian to obtain required intake/enrollment documents.
Completes all necessary admission paperwork for enrollment in CES Waiver services.
Maintains current knowledge eligibility, including funding, and licensure requirements (DHS/DDS, DPSQA, PASSE) for CES.
Arrange intake and individual program development meetings with individual served, parent/guarding, PASSE care coordinator and ESA staff.
Maintains information in databases and prepares quarterly and annual service statistics reports.
Provides weekly reports on status of referrals and participates in enrollment meetings.
Assists in the development of the initial individual program plan, schedule/calendar, and budget including request for prior authorization.
Provides support to department as assigned by supervisor.

While performing the duties of this job, employee will comply with all federal, state and local laws and regulations pertaining to Easterseals Arkansas.
Employee has access to all protected client health information in CES Waiver.

Education and/or Experience:
High School Diploma/GED and 3-5 years’ experience in providing administrative services in a program that supports individuals with an Intellectual Developmental Disability, or other Social Services or Health Related program. A bachelor’s degree in social services or another Health Related Field preferred.
