Job Overview:
We are seeking a dedicated and compassionate Resident Service Coordinator to join our team. As a Resident Service Coordinator, you will play a crucial role in coordinating and managing services for individuals with mental health and developmental disabilities. Your primary responsibility will be to ensure that all clients receive the necessary care and support to enhance their quality of life.
Duties:
- Coordinate and manage services for individuals
- Conduct assessments to determine client needs and develop individualized care plans
- Collaborate with healthcare professionals, caregivers, and agencies to ensure comprehensive care
- Monitor client progress and make necessary adjustments to care plans as needed
- Provide case management services, including coordinating appointments, referrals, and follow-ups
- Assist with meal preparation, medication management, and personal care as required
- Maintain accurate and up-to-date documentation of client records
Skills:
- Strong knowledge of case management and individualized care
- Experience working with individuals with mental health and developmental disabilities or similar populations
