East Bay Community Action Program (EBCAP) is seeking a full time Intake Coordinator for our Health Center at 100 Bullocks Point Avenue in Riverside RI.

The Intake Coordinator will work with the health center clients and perform various screenings to determine client needs with respect to healthcare and/or social determinants. Refer to appropriate services internally and externally as needed. Provide specific assessments to Health Center clients to determine individual client needs. Assessments may include, but are not limited to social history, PHQ (patient health questionnaire), Cage Questionnaire (“cut, annoyed, guilty, eye” – identify problems with alcohol), Tobacco Usage, GPRA (“government performance & results modernization” – collect, improve, report outcomes), Social Determinants, etc.

The Intake Coordinator must have a minimum of a High School Diploma or equivalent. Prior customer service skills required.
