DuPagePads is Hiring a Street Outreach Case Manager Near Wheaton, IL
Responsibilities: (to be performed with or without reasonable accommodation):
- Operates within the DuPagePads Core Values, which supports providing care in accordance with the Trauma Informed Policy and Practices of the Agency.
- Work with the team to engage and provide outreach, crisis intervention, and assessment and case coordination to unsheltered individuals.
- Monitor and act on community reports of unsheltered individuals.
- Establish relationships with community police departments, libraries, and community organizations for the purpose of identifying individuals who need services.
- Provide screening and intake of program participants who are experiencing unsheltered homelessness.
- Provide ongoing case management services including linkage to mental health care, employment, medical care.
- Assist with applications for public benefits
- Provide appropriate referrals to the Interim Housing Sites
- Engage with program participants to build relationships to help move them to the next level of service.
- Document and report any hospital calls or emergencies to the Street Outreach Supervisor.
- Provide required program forms and information to the Street Outreach Supervisor and/or office staff according to expected deadlines.
- Maintain required data entry according to expected deadlines.
- Attend and actively participate in all required meetings and trainings
- Other duties as assigned by supervisor.
- Excellent engagement skills.
- Effective written, oral, and crisis intervention skills.
- Ability to think conceptually and to be creative.
- Strong interpersonal skills with attention to detail.
- Problem solving skills.
- Ability to plan and analyze.
- Ability to work independently and work as part of a team.
- Experience in issues related to homelessness, mental health/substance abuse preferred.
- Computer literacy in Microsoft Office.
- Bachelor’s degree in Social Work preferred, or equivalent work experience.
- Strong customer service focus/ positive attitude.
- CADC or CARS Certification a plus
- Experience in issues related to homelessness, poverty, and mental health preferred.
- Willing to work a flexible schedule including evenings, weekends, and holidays.
- Provide own transportation.
- Must possess valid Illinois Drivers' License
