Care Specialist – Interim Housing
Summary: This position supports adults with behavioral health, intellectual, and/or developmental disabilities, applying a range of interventions to support each client’s wellness, participation, and outcomes. Clients are assigned based on level of need, and the unique role may provide services across our range of programs. Duties include, but are not limited to, providing street outreach and access services, case management, and goal plan to develop independent living, symptom management, coping, social, and housing retention skills.
Reports To: Supervisor, Access Services
Responsibilities: (To be performed with or without reasonable accommodation):
  • Operates within the DuPagePads Core Values, which supports providing care in accordance with the Trauma Informed Policy and Practices of the Agency.
  • Provides outreach and access services, crisis intervention, assessment, and care coordination to unsheltered individuals.
  • Monitor and act on community reports of unsheltered individuals.
  • 2Assist clients with accessing shelter and housing through agency programs, other community programs, and warming centers.
  • Develop care / goal plans with assigned clients that improve the client’s wellbeing and ability to obtain and sustain temporary or permanent housing.
  • Implement interventions per the goal plan that supports the development of independent living, coping, symptom management, social and other functional skills.
  • Communicate assessment information, care plans, and other relevant information to team members to ensure consistency in client interventions.
  • Support clients in accessing community resources, accompanying clients as needed.
  • Utilize best practices including Motivational Interviewing, Housing First, and Harm Reduction.
  • Document all services and data elements while adhering to accuracy and timeliness standards.
  • Advocate and provide increased community education, relationships, and supports to benefit unsheltered individuals.
  • Attend and actively participate in all required agency and community meetings.
  • Other duties as assigned by supervisor.
Knowledge, Skills and Abilities:
  • Strong engagement and interpersonal skills.
  • Demonstrable warmth, empathy, and a nonjudgmental disposition.
  • Experience in person-centered, trauma-informed interventions.
  • Ability to be creative, think conceptually, plan, analyze, and apply problem solving skills.
  • Effective written and oral communication skills, including crisis intervention skills.
  • Ability to work independently and work as part of a team.
  • Computer literacy.
Qualifications:
1. Bachelor’s degree in social work, human development, psychology, or related field with experience serving individuals with behavioral health, intellectual, and / or developmental needs.
2. Knowledge of best practices in Motivational Interviewing, Harm Reduction, Housing First, life skills teaching, and areas of recovery are preferred.
Other:
1. Weekday, daytime hours are normal, flexibility is required to meet needs of clients.
2. Provide on call coverage according to assigned work schedule.
3. Provide own transportation.
Classification Status: Full-time non-exempt
Starting pay: $21.00
