Dunson and Associates is seeking a RNs to join our team! You will be responsible for the assessment, diagnosis, and treatment of assigned patients.

Job Summary: As a RN Assessor, you will conduct comprehensive assessments for elderly and/or disabled individuals in Baltimore City, Maryland, at risk of nursing facility placement. You will play a crucial role in helping these individuals remain independent and live in the least restrictive setting possible by identifying their needs, connecting them to resources, and developing person-centered plans of care.
Job Responsibilities:
· Conduct thorough in-home assessments of clients, focusing on their medical, functional, psychosocial, and environmental needs.
· Utilize standardized assessment tools and interview techniques to gather information about clients' health history, daily living skills, support systems, and financial resources.
· Analyze assessment data to identify strengths, challenges, and service needs.
· Collaborate with nurses, physicians, and other professionals to develop comprehensive plans of care that promote independence and community living.
· Connect clients to appropriate community resources, such as home care services, transportation assistance, and support groups.
· Advocate for clients' rights and access to services.
· Provide accurate and detailed documentation of assessments, plans of care, and service referrals.
· Participate in team meetings and training sessions.
· Stay informed about changes in policies and procedures related to Client and Medicaid programs.
Education and Experience Requirements
· At a minimum, a Bachelor's degree in Social Work
· Holds, at minimum, a licensed bachelor's social worker (LBSW) license.
· Minimum 2 years of experience in medical-surgical/geriatric and/or pediatric case management.
Skills and Knowledge Requirements
· Must be proficient in computer applications including Microsoft Office Suite, Microsoft Teams, Microsoft 365, and electronic medical records (EMR) systems; experience with LTSS system preferred
· Strong clinical skills and assessment expertise.
· Excellent communication, interpersonal, and collaboration skills.
· Proficiency in written and verbal communication.
· Ability to work independently and manage multiple priorities.
· Culturally competent and committed to providing equitable services.
· A valid driver's license and reliable transportation are required.
