The Duke Dementia Family Support Program offers education, support, and engagement for those impacted by memory disorders.

General Description

The social worker will provide education and support to people living with dementia and their families around issues related to adapting to a dementia diagnosis, living with memory loss, and caring for someone with dementia. This work will be with individuals and groups, working at times individually and at other times as part of a team effort.

Location:This rolefollows a hybrid schedule working in the office three days a week and remotely two days (includes some travel within Central NC). Our offices are in the Center for Aging in Duke South Clinic, Durham.

Work Hours: Daytime Monday-Friday with limited evening and weekend hours required monthly to facilitate support groups.

Job Description

  1. Information and Assistance: Respond to individual telephone and e-mail consultations from NC families seeking services and information about memory disorders.
  2. Project C.A.R.E.: Serve as Lead Family Consultant for Project C.A.R.E. (Caregiver Alternatives to Running on Empty) in the 19 counties of Central NC. Family Consultant responsibilities include:
    1. Conduct home visits to enroll new clients. This requires use of your personal vehicle and mileage will be reimbursed.
    2. Provide persons with dementia and their family caregivers counseling, care consultation, dementia-specific information, caregiver assessments, caregiver education and connections to strong social support networks.
    3. Award Project C.A.R.E. respite vouchers
      1. Assess need for respite care
      2. Assist families in determining how to find and use respite care
      3. Complete requisite voucher paperwork and online spreadsheet
      4. Track voucher use and follow-up with caregivers
    4. Enroll caregivers into Project C.A.R.E. and enter information into ARMS (State database).
    5. Coordinate Central NC Project C.A.R.E. clients with other DDFSP team members
    6. Manage reports to State
  3. Support Groups and Outreach Programs:
    1. Co-facilitate monthly support groups and outreach programs for individuals living with dementia and family care partners, including the African American Dementia Care Partner Support Group. Evening or Saturday hours required monthly.
    2. Recruit, screen and co-facilitate Memory Makers: An Early-Stage Memory Loss Educational Support Program for people living with dementia and their care partners.
    3. Participate in preparation and distribution of communication and publicity for above-mentioned programs.
    4. Help design and implement new groups, programs, and services as needed.
    5. Help develop assessment tools for evaluating program outcomes.
  4. Organize content and manage layout (iContact) for the Triangle Monthly E-News.
  5. Outreach and Education: Create and offer brief educational programs on dementia and caregiving for families and professionals.
  6. Participate in collaborative efforts with state, regional and national organizations.
  7. Office management: Assist staff in completing day-to-day office tasks, including, but not limited to: responding to inquiries on the main phone line and program email and entering data in our client tracking system REDCap and iContact.
  8. Website and social media
    1.  Participate in updating website (knowledge of WordPress helpful)
    2.  Assist with Facebook posts and help expand social media presence.

Knowledge Skills and Abilities

  • Adherence to the practice standards and ethical guidelines of NASW and the NC Certification Board for Social Work.
  • Excellent verbal and written communication skills.
  • Ability to take initiative and a willingness to learn.
  • Ability to work well both in teams and independently.
  • Ability to prioritize multiple tasks and manage time efficiently.
  • Basic computer skills and willingness to learn database and website software used by DDFSP.
  • Knowledge of dementia preferred.
  • Experience working with care partners and the individual living with changes in memory and thinking preferred.

Education

  • Master's degree in social work from an accredited school of social work.

Licensure

  • Current licensure as a licensed clinical social worker (LCSW) or associate (LCSWA) by the NC Social Work Certification and Licensure Board preferred
