REQUIREMENTS:

• Must be at least 22 Years of Age
• SAP VIOLATION MUST BE A YEAR OLD
• NO REFUSALS
• Must have taking the RTD test and Clearinghouse must say 'Not Prohibited'
• Follow up testing has to be completed. (1 or 2 left is ok)
• 2 years of experience of Class A Driving
• No more than 3 MV's in the last 3 years
• No more that 1 accident in the last 3 years
• No DUI/DWI in the last 5 years
• No speeding ticket 26 over in the last 5 years
• DRIVER MUST PROVIDE DOT MEDICAL LONG FORM
• Clean criminal background for the last 10 years
