Doorways of Northwest Florida is a Homeless Solutions nonprofit provider serving as the Continuum of Care Lead agency for Bay, Calhoun, Gulf, Holmes Jackson and Washington Counties in Florida. Our mission is to prevent and end homelessness.

The purpose of case management is to assist individuals in gaining access to resources and benefits that will support permanent housing stability.

The primary goal of the case manager position is to optimize the functioning of individuals who have complex needs by coordinating the provision of support services in the most efficient and effective manner. The desired outcomes for persons using the service system are self sufficiency, housing stability and resiliency.

The case manager provides support to the individual through assessment, monitoring, linkage, coordination, referral, and advocacy. Case Managers will create and maintain case file documentation; assist with accessing main stream benefits; building budgets and other life skills; with the ultimate goal of securing safe, affordable housing that can be maintained independently.

There will be on-site and in office visits and outreach to homeless individuals and families.

Pay: $37,000.00 - $42,000.00 per year
