Divine Rehabilitation and Healthcare at Sylvania is looking to hire a talented Unit Manager! - Great pay and benefits!

Duties and Responsibilities:

  • Must have critical thinking ability must be able to determine appropriate solutions to issues as they arise. Must be able to remain calm in emergency situations.
  • Walking rounds of unit and staff huddles.
  • Ability to work every other weekend to ensure continuity of care and compliance with regulatory requirements of assessment completion order implementation in a timely manner upon admission.
  • Leading care conferences and driving successful outcomes in care.
  • Daily review of alert charting/progress notes, follow up on labs & cultures, and tracking of infections and/or treatment. Reviewing 24-hour report daily to ensure adequate responses to: changes in condition, transfers, discharges, use of physical or chemical restraints, incident, injuries, potential abuse or neglect, medication errors, loss of resident property, and/or resident or family dissatisfaction. Insures adequate investigation, documentation, notification/reporting, and resolution of all of the above.
  • Assisting new residents transition to post-acute care setting and ensuring completion of admission paperwork and coordination of necessary external appointments.
  • Managing risk, skin, infection, psychotropics and other general chart audits to ensure quality compliance.
  • Ensure that facility is maintaining appropriate wound care and required documentation for all applicable residents.
  • Responsible for overseeing the care for residents residing in assigned unit(s) of the facility by supporting and practicing the philosophy, nursing objective and standards of the nursing department along with State and Federal regulations.
  • Staff supervision & perform training, counseling and disciplinary action for the unit nursing staff.
  • Participates in the implementation of the care planning process to comply with resident needs and regulatory requirements.
  • Responsible for collecting data on current infections, tracking initiation and completion of ATB therapy, implementing isolation when applicable, tracking infection trends and utilizing the information to educate staff on appropriate preventative measures.
  • Be able to interact positively and tactfully with residents, families, and other healthcare professionals at all times.
  • Qualified candidates must have a valid Ohio Nursing License.

Required experience:

  • Supervisory experience in a Skilled Nursing facility
  • Active Registered Nurse (RN) license or Licensed Practical Nurse (LPN) license in the State of Ohio
