Salary: $32.43/hr ($67,470/annually)

Position Summary:

We are seeking caring, compassionate, mission driven Intensive Case Managers to provide Intensive Case Management (ICM) services, wraparound facilitation, and supportive interventions to families with dependent children identified by the school system as living homeless or housing unstable including doubled-up or in hotel/motel. This position is designed to expressly prevent literal homelessness when possible and when homelessness has already occurred to ensure that the time homeless is brief and non-reoccurring. It includes assessing, linking, coordinating and monitoring services by addressing social determinants of health including mental health. This position works in collaboration to develop service plans in partnership with the family, works closely with the family to identify services needed in the home to maintain a stable living environment. Services are provided in-home and in the community. The ICM works closely with the family to connect with resources and offer educational, financial or other assistance as needed or appropriate. The ICM attends and participates in the Integrated Decision Team shared risk model. The ICM transport clients in personal vehicle as part of service provision. The ICM completes documentation per DFL's policy related to services provided. The ICM responds to crisis situations in a professional and timely manner and apprises supervisor. The Intensive Case Manager provides approximately 3.5 hours per week minimum serving each assigned family, and will increase contact hours based on family dynamics, situation, and risk factors. DFL prides itself on the fact that it is a trauma informed care services provider. This position requires that trauma informed practices are embedded into all services provided. Hours are flexible, evening and weekend work is required. The ICM will be required to become a high Fidelity Wraparound Facilitator within 6 months of accepting the position.

EDUCATION EXPERIENCE/REQUIREMENTS:

  1. Must hold at least a Bachelor's Degree Social Work or related area of study from an accredited university; or a Master's Degree in Social Work or related area of study.
  2. Must have at least two (2) years' experience working with vulnerable children and families including those whom live homeless or housing unstable.
  3. Must be able to rapidly acquire knowledge of community resources.
  4. Must be computer literate and be proficient in Outlook, Excel spreadsheets, and Microsoft Word. Must be able to work proficiently in a Windows environment and be able to learn other software applications quickly. Must have the ability to learn the PHMIS within the first 30 days in the position. This is a condition of employment.
  5. Must have reliable transportation and be able to use personal vehicle to transport clients. Must have and maintain clear driving record, must have valid Florida Driver License, maintain required automobile insurance coverage and pass the annual vehicle inspection.
