Direction Service is Hiring a Service Coordinator Near Springfield, OR

Description

Service Coordinators support families of youth with disabilities: locates resources, communicates effectively with community resource partners, schools, and other service systems, supports clients to navigate school systems, and supports clients to problem-solve barriers to needed/desired supports and services. This job is typically full-time, and the general schedule is Monday-Friday from 8:00 AM to 4:30 PM. Part-time positions are available as well.

Pay Differentials:

Pay differentials are given for bilingual (Spanish) language skills and/or advanced degrees (master's and above) in required field.

Essential Functions of the Job: To perform this job successfully, an individual must be able to perform each essential job function. Reasonable accommodations may be made to enable qualified individuals with disabilities to perform the essential job functions.

  • Provides service coordination and advocacy services to clients, including: assessing needs and strengths, assisting understanding of medical and educational needs, developing short- and long-term goals, and obtaining appropriate services.
  • Helps clients understand and assert their rights and responsibilities under a 504 plan, IEP, or IFSP.
  • Identifies options and provides information to facilitate the clients’ abilities to make informed decisions.
  • Coordinates, attends (as able), and facilitates meetings between clients and providers.
  • Facilitates conflict resolution and communication between agencies, school districts, and clients.
  • Provides on-going support and follow-up to clients.
  • Makes appropriate referrals to resources and services in the community that supports the client’s needs and/or goals.
  • Supports clients to access transition services.
  • Maintains accurate and detailed files/records including time tracking, mileage, client tracking, and case notes.
  • Collaborates with other Youth and Family Service staff to address issues of common concern to the program.
  • Maintains and increases knowledge through reading, workshops, and other resources.
  • Maintains a flexible work schedule.
  • Communicates positively with clients, families, and community partners.
  • Travels to clients’ homes and community locations when needed to serve clients and/or represent Direction Service.
  • Empowers clients to advocate for their child and family by providing them with information and models positive advocacy and problem-solving.
  • Participates in fundraising, development, and outreach activities.
  • Performs other related duties as required and assigned.

Requirements

Required Education & Experience: 

  • A bachelor's degree in education from an accredited institution or combination of three years' experience in a related field. 
  • Working knowledge of school related practices and organizations as well as experience working in or with schools: public, private, charter, and/or alternative is required.

Preferred Education & Experience: 

  • Master's degree in education, preferably special education from an accredited institution. Working knowledge of school-related practices and organizations. Experience working in or with schools: public, private, charter, and/or alternative. 
  • Bilingual in Spanish

Other Required Qualifications: 

  • Driving is an essential function for this position. Must have own reliable transportation for travel throughout Lane County. Must also possess a valid driver’s license and an acceptable driving record. Employee is required to maintain insurance on their vehicle in accordance with Oregon law. 
  • Employment contingent on a satisfactory Criminal History Check as required by the State of Oregon (OAR 407-007-0200 to 407-007-0370) and as deemed acceptable by Direction Service.

Working Conditions / Physical Demands:

  • Majority of work is completed in an office setting with exposure to moderate levels of noise in a well-lit and moderately paced environment. Continuously required to sit, stand, walk, hear and/or respond to verbal/audio cues, see and/or respond to visual cues perform repetitive motions of hands and wrists, and read computer screen. Additional physical requirements may include reaching with hands and arms, stooping, kneeling, crouching, crawling, climbing up stairs and walking up inclines. 
  • Exposure to communicable diseases found in children and the public.
  • Ability to cope with clients who may use challenging or offensive language with the potential for escalation to physical aggression.
