Description

Assists care managers with providing care management for managed care enrollees in the LTSS division

• Evaluate assessment and care management data to determine ongoing program eligibility, care needs and service options.
• Understand how to make Level of Care determinations for consumers as part of initial, annual and event-base (if applicable) assessment process
• Monitor and evaluate the care plan, service quality and program eligibility
• Collaborate with the Department of Job and Family Services to establish and/or maintain Medicaid eligibility
• Assist with completing the enrollment process of eligible consumers into the program
• Provide education and support to consumers and their caregivers
• Understand how to develop and implement an individualized care plan for all consumers
• Advocate on behalf of consumer and/or caregiver/family and assist individuals in securing appropriate services and care settings
• Assist with disenrolling ineligible consumers from the program
• Apply judgement and perform clinical practice based upon education, experience and professional practice standards.
• Participate in and contribute towards accomplishment of divisional and Agency dashboard goals

Qualifications

• Interested in developing an understanding of care management roles and responsibilities
• Interest in working with the elderly and disabled population

Physical/Mental Position Qualifications

• Ability to effectively communicate with the elderly and disabled population and general public, both in person and by phone,
• Ability to comprehend written material related to essential job functions,
• Ability to accurately document facts related to essential job functions,
• Ability to operate a computer well enough to perform essential job functions,
• Ability to comprehend, recall, and apply facts related to essential job functions,
• Ability to analyze, evaluate, and implement a reasonable course of action based on available information,
• Ability to detect specific odors related to the assessment of a client's health/hygiene and the safety of the client's environment,
• Ability to legally drive a car or make other transportation arrangements, and
• Ability to ambulate enough to access private homes/locations which may not be readily accessible.
