Job Summary: The Hospice Admissions Nurse plays a crucial role in the hospice care team and is responsible for various tasks related to the admission process of patients. They will be the primary person to respond when a referral is posted on the board and coordinate with the family, patient, and facility to set appointments.
Qualifications/Educational Requirements:
Responsibilities/essential functions: The person in this position must be able to perform the following essential job functions with or without reasonable accommodations:
Working Conditions: General offices, extended care facility, patient home setting, and exposure to infectious diseases, travel by automobile.
Physical Demands: Minimal physical exertion. Regularly required to use hands, talk and hear. Specific vision abilities include close, peripheral, depth perception, and ability to adjust focus. Must be able to travel to businesses, offices, or patient homes or where conditions may be adverse due to weather, environment, or patients’ lifestyles.
Continuing Education Requirements: Employee is expected to participate in continuing education as required by Medicare or ACHC standards. In addition, employee is expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities. Employee must virtually attend mandatory education programs.
Scheduling & On-Call: Nurses are expected to work 2 “Holiday” shifts in a 12-month period.
