Job Summary: The Hospice Admissions Nurse plays a crucial role in the hospice care team and is responsible for various tasks related to the admission process of patients. They will be the primary person to respond when a referral is posted on the board and coordinate with the family, patient, and facility to set appointments.

Qualifications/Educational Requirements:

  • Must possess current, unencumbered, active license to practice as a Registered Nurse (RN) in the State of Colorado.
  • Must have valid car insurance and driver’s license.
  • Knowledge of and experience in pain and symptom management.
  • Well-developed physical assessment skills.
  • Must be able to read, write, speak, and understand the English language.
  • Must possess patience, tact, and a positive attitude.
  • Minimum of one year nursing experience in a hospice setting.
  • Excellent organization and communication skills.
  • Excellent interpersonal and analytical skills.

Responsibilities/essential functions: The person in this position must be able to perform the following essential job functions with or without reasonable accommodations:

  • Managing all aspects of a referral by coordinating with families, patients, and communities to set appointments.
  • Conduct comprehensive assessments of patients referred to hospice.
  • Determine eligibility by working with Dignity’s NP and Medical Physician and the patient’s healthcare providers.
  • Completing consents and obtaining the MOST and MDPOA paperwork, etc.
  • Collaborate with the hospice interdisciplinary team to create a care plan.
  • Ensure the patient’s medications are reviewed and adjusted as needed to provide effective symptom management and pain control.
  • Order necessary DME if needed.
  • Make referrals to other team members as needed to support the patient and family (e.g., chaplain, social work, bereavement counselor, etc.)
  • Arranging transportation if necessary.
  • Offer emotional support and empathy to patients and their families during this challenging time, helping them navigate the emotional aspects of end-of -life care.
  • Conduct the RN Assessment.
  • Always use discretion with patient information and act in a professional manner.
  • Participate in staff meetings, in-service programs, and Utilization reviews.
  • Document communication, visits, and related clinical actions in EMR within 24 hours of activity.
  • During times of urgent admissions, the Admissions Nurse will delegate referrals to the RNCM.
  • Travel Required: Expectation is to respond to all referrals for Dignity Hospice (from Fort Collins, down to Castle Rock).
  • Be prepared to help as a float RN during the lull of admissions.
  • Other duties as assigned.

Working Conditions: General offices, extended care facility, patient home setting, and exposure to infectious diseases, travel by automobile.

Physical Demands: Minimal physical exertion. Regularly required to use hands, talk and hear. Specific vision abilities include close, peripheral, depth perception, and ability to adjust focus. Must be able to travel to businesses, offices, or patient homes or where conditions may be adverse due to weather, environment, or patients’ lifestyles.

Continuing Education Requirements: Employee is expected to participate in continuing education as required by Medicare or ACHC standards. In addition, employee is expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities. Employee must virtually attend mandatory education programs.

Scheduling & On-Call: Nurses are expected to work 2 “Holiday” shifts in a 12-month period.
