Overview:
Diamond Willow is an Assisted Living and Memory Care facility that started in 2004. Located in Cloquet, MN, Diamond Willow is dedicated to providing quality care to our twenty-six residents through compassionate care and assuring that our residents and their needs remain at the forefront of our focus.

Diamond Willow has partnered with Aspire Clinical Intelligence (clinical on call help center for care staff) to help support our ongoing clinical needs when our Registered Nurses are off-site. This means more time for our RN’s to recharge and give back to our residents and staff. Imagine doing what you love but still getting the time off that you need. Very minimal time on-call during the weekdays and weekends.

Our Detroit Lakes, MN location is seeking a .06 or.08 Case Manager/Clinical RN to support our RN Supervisor and Team. This is an awesome opportunity for a new RN graduate or a seasoned RN who is looking for more work life balance. We are willing to be flexible on days you work!!!!

Responsibilities:
Observing, tending to, and documenting resident's health needs including appts., labs, etc.
Communicating with Administrator regarding relevant staff and resident information and/or concerns
Completing Vulnerable Adults, MAARC Reports, and Incident Reports
Ensure MD orders are processed and performed
Updating families on any changes to the resident and addressing family or patient concerns.
Providing tours for potential admits and admitting new residents.
Addressing and documenting any staff issues that arise.
Overseeing staff and ensuring all staff are meeting job expectations and requirements.
Med management 

Requirements:
Passionate about leading a team to high levels of performance in providing outstanding care to our residents
Will need to ensure staffing is adequate and meets our resident’s needs- this could mean minimal time spent onsite on various shifts and weekends
Maintaining confidentiality and professionalism at all times
Must be able to work as a team member and maintain a positive demeanor
Ability to be on call from 7am-7pm on weekdays, limited weekend on-call.

Qualifications:
Possess a valid Minnesota Registered Nurse (RN) license
Assisted Living Experience preferred
