Physical Therapist agrees that the provision of his/her service and the care and products he/she provides shall, at all times, be subject to the control and supervision of the Agency. The Agency will coordinate the services provided by the PT and the PT shall assist and cooperate with respect to this. Such cooperation shall include but not be limited to attendance at Case Conferences.
Physical Therapist agrees that he/she shall attend Agency orientation (and sign and Orientation form) and be subject to a competency evaluation prior to making patient visits. Additional competency evaluations shall take on an annual basis. Physical Therapist further agrees that he/she shall be subject to a Performance evaluation three months after hire and then annually.
Physical Therapist shall contribute to the Agency’s performance Improvement initiative by virtue of the activities outlined in paragraphs A, C, D, E, F, G and H below. The Agency may contract with more than one Physical Therapist. At least one Physical Therapist shall be sitting member of the Agency’s Professional Advisory Committee, Quality Improvement Committee and Clinical Record Review Committee.
Physical Therapist shall sign a “Business Associate Agreement” under which he/she agrees to comply with HIPAA privacy rules.
